Form 1095-B is used to report information about minimum essential health coverage to the Internal Revenue Service. CSAC has already provided your information to the IRS. It is not necessary to print and mail Form 1095-B with your tax return.

To request a printed copy of your Form 1095-B, please call (213) 386-8590 or toll-free (800) 293-1370 during business hours (Monday-Friday, 8 a.m. to 5 p.m. Pacific).
